WHAT IS THE NEW PATIENT EVALUATION? The new patient evaluation is the perfect opportunity for a new patient to establish care with our providers. This visit typically takes 30-45 minutes as the practitioner will ask many questions to learn about each patient’s medical history, family history, and health concerns. The goal of the evaluation is to establish a relationship between the patient and the provider as well as create mutual personalized health goals in order to further the journey to optimal wellness. WHO SHOULD BOOK A NEW PATIENT EVALUATION? New patients who have never been seen at our office before should schedule a new patient evaluation. WHAT SHOULD I BRING TO THE EVALUATION? Patients should bring any pertinent health information that they physically possess, such as a list of medications and dosage, dates of previous surgeries, and other relevant health records.
